Singapore Medical Treatment STVP Extension 2026: ICA Requirements, Form V75 & Fees

A foreign visitor who needs to remain in Singapore longer for medical treatment can apply online to ICA for an extension of the Short-Term Visit Pass (STVP). The current ICA extension page, updated 30 June 2026, states that an STVP extension application can be submitted when the existing STVP has 14 days or less of validity remaining. ICA assesses each application holistically and the period granted is subject to ICA's assessment; there is no current official rule saying that every medical-treatment visitor can extend to exactly 89 days total. For a medical-treatment extension, ICA may require a completed Form V75 endorsed by a registered doctor in Singapore, stating the illness and the period of stay required. A local sponsor is required only in specified cases, including where the visitor has immediate family members in Singapore who are Singapore Citizens or Permanent Residents, or where the visitor is a national or passport holder in one of the categories listed by ICA. The sponsor must be a Singapore Citizen or PR aged at least 21 with Singpass. The current ICA page also states that an extension fee of S$40 is payable only when the approved extension causes the visitor to remain in Singapore for 90 days or more from the date of entry or issuance of the pass, and for each subsequent 90-day block. Visa fees of S$30 can also apply to visa-required visitors. Approved extensions are issued electronically and do not require a physical endorsement.

What is the medical-treatment STVP extension?

Singapore does not issue a separate general 'medical visa' for foreign patients already in Singapore on a Short-Term Visit Pass. Instead, ICA provides an extension-of-stay process for visitors whose circumstances require additional time in Singapore. One expressly recognised circumstance is medical treatment. The visitor remains subject to the conditions of the STVP and may not work, conduct business or engage in a profession or occupation unless otherwise authorised by a valid work pass or applicable exemption.

There is no guaranteed 89-day medical-treatment limit

The original page's statement that a foreign medical patient can extend a standard 30-day tourist stay to 'up to 89 days total stay' is not supported by the current ICA extension page. ICA states that it considers each application holistically, taking account of factors such as the circumstances surrounding the request and whether the visitor has immediate family members who are Singapore Citizens or Permanent Residents. The period of extension granted is subject to ICA's assessment. The current page therefore does not establish a universal 89-day maximum for medical-treatment cases.

When should a foreign patient apply?

ICA's current extension page states that visitors may submit an STVP extension application when the STVP has a remaining validity of 14 days or less. This replaces the original page's unsupported recommendation to apply at least 7 days before expiry. All STVP extension applications must be submitted online and walk-in applications are not accepted. Applicants should not wait until the final day because the stated processing time is up to five working days and some applications may take longer.

Form V75: medical evidence for an extension on medical grounds

For visitors seeking an extension for medical treatment, ICA's current guidance states that additional documents may be required and gives Form V75 as the relevant medical document. Form V75 must be duly endorsed by a registered doctor in Singapore and should contain details of the illness and the period of stay required. The form supports ICA's assessment; it does not itself grant a particular number of additional days. The doctor should therefore provide clinically supported information rather than simply writing a requested immigration duration without medical justification.

Is a Singapore Citizen or PR local sponsor always required?

No. The original page's blanket sponsor requirement is too broad. ICA's current STVP extension rules require a local sponsor only in specified circumstances. These include visitors who have immediate family members in Singapore who are Singapore Citizens or Permanent Residents, and visitors who are nationals of, or holders of passports or travel documents from, the countries or categories specifically listed by ICA. The listed nationalities include Afghanistan, Algeria, Bangladesh, Egypt, Iran, Iraq, Jordan, Kosovo, Lebanon, Libya, Mali, Morocco, Nigeria, Pakistan, Somalia, South Sudan, Sudan, Syria, Tunisia and Yemen, as well as certain Palestinian Authority, UAE temporary-passport and Middle-East refugee-travel-document holders. Where a sponsor is required, the sponsor must be a Singapore Citizen or PR who is at least 21 years old and has a Singpass account.

Documents required for the STVP extension

ICA's current online process requires a valid travel document, such as a passport, and the Disembarkation/Embarkation (D/E) number from the SG Arrival Card. Depending on the circumstances, ICA may request additional supporting documents or clarifications. For medical-treatment extensions, this can include the completed Form V75 endorsed by a registered doctor in Singapore. Applicants with immediate family members in Singapore may also be asked for original documents proving the relationship. Where a local sponsor is applicable, the sponsor provides the required information through the e-Service using Singpass.

How much does the ICA STVP extension cost?

The S$40 charge is not a universal application processing fee payable by every applicant when submitting the request. ICA's current fee rule states that an extension fee of S$40 is applicable if the extension granted results in the applicant staying in Singapore for 90 days or more from the date of entry or issuance of the pass. The same S$40 fee applies for each subsequent 90-day block. For example, another S$40 can become payable when the approved stay reaches 180 days or more. A separate S$30 visa fee may be payable for visa-required visitors. The stay extension is only granted once any applicable payment has been made.

What happens after ICA approves the extension?

ICA sends the application outcome by e-notification to the email address provided in the application. For approved online extensions, no physical endorsement is required. Applicants should verify the newly granted validity through ICA's visit-pass validity tools or e-Pass enquiry service and should ensure that they leave Singapore on or before the new expiry date. If an application is not approved before the existing STVP expires, the visitor should not assume that the pending application itself authorises an overstay.

How long does ICA take to process an extension?

ICA states that an STVP extension will be processed within five working days, excluding public holidays and weekends. Some applications may take longer. Medical-treatment applications can require additional documents or clarifications, so applicants should not plan treatment-related travel around an assumed guaranteed decision date. The five-working-day statement is a processing target published by ICA, not a guarantee that every case will be decided within exactly five working days.

Can a caregiver or family member stay with the patient?

A patient and a companion are assessed under their own immigration status. The fact that one person has a medical-treatment reason does not automatically confer an extension on another family member. A caregiver or family member who also needs additional stay should check whether they independently qualify for an STVP extension and whether ICA requires a separate application or supporting evidence. The original claim of an automatic 'concurrent caregiver pass extension' is too broad and should not be presented as a guaranteed benefit.

Healthcare costs and insurance for foreign patients

Foreign visitors should not assume that Singapore's public healthcare subsidy framework will pay their medical bills. Non-residents generally do not receive the standard public healthcare subsidies available to Singapore Citizens and Permanent Residents, subject to service-specific rules and exceptions. Hospital deposits, guarantees of payment and insurance acceptance depend on the institution and insurer. ICA's current STVP extension page does not make proof of medical insurance or proof of sufficient hospital funds a universal document requirement, so these should not be presented as mandatory ICA requirements for every medical-treatment extension. Nevertheless, patients should arrange payment or insurance with the treating hospital before major elective treatment because the immigration extension does not itself reduce the medical bill.

Medical treatment does not change the normal STVP restrictions

An STVP remains a short-term visit status. ICA states that STVP holders must not engage in employment, paid or unpaid, business, profession or occupation in Singapore unless they hold a valid work pass or are otherwise engaging in an authorised work-pass-exempt activity. A medical-treatment extension therefore allows additional lawful stay for the approved purpose; it does not turn the visitor into a worker, resident or long-term pass holder.
