If you are eligible for the 'Simplified Renewal' program, you do not need a guarantor to sign your photos, but you must still provide two references who have known you for at least 2 years. If you do not meet simplified renewal criteria (e.g. passport expired for over 15 years, or passport lost/damaged), you must submit the general application form (PPTC 040) which requires a qualified guarantor (a Canadian citizen holding a valid 5-year passport who has known you for 2 years). Mail the physical package via a secure trackable mail service (USPS or FedEx) to: Passport Program, 22 de Varennes Street, Gatineau, Quebec, J8T 8R1, Canada. Your new and old passports will be mailed back to your U.S. address via Canada Post/USPS.

Always make full-color digital scans of your current passport data pages and valid U.S. visa stamps before mailing your physical documents anywhere. Consulates and courier services occasionally lose packages, and having digital backups of your identification numbers will greatly speed up the process of issuing emergency replacement documents.
Ensure that all checks, cashier's checks, or money orders are made out to the exact spelling of the consular office or authorized vendor (e.g., 'VFS Services USA Inc.'). Minor typos on check lines will result in the immediate return of your entire application package.
