What to Do After Reporting the Passport Lost or Stolen

Once the Department of State cancels a valid passport reported lost or stolen, that passport is no longer valid for travel even if you later find it. Apply for a replacement using the current passport process. If you are in the United States, use the appropriate DS-11 procedure unless you qualify for another authorized replacement or renewal method. If you are abroad, contact the nearest U.S. embassy or consulate and follow its local instructions.

A valid lost or stolen passport must be reported to the Department of State. You can report it online, by mail using Form DS-64, or while applying for a replacement with Form DS-11. A replacement passport generally requires Form DS-11 and an in-person application when you are applying in the United States. If all relevant loss or theft information is provided on DS-11, the Department may not require a separate DS-64.

A lost or stolen passport is not eligible for ordinary DS-82 renewal. In the United States, you generally apply in person using Form DS-11. Eligible adults can use DS-82 for renewal only when they otherwise satisfy the renewal requirements, including having the passport available and not reporting it lost or stolen.

Contact the nearest U.S. embassy or consulate immediately. If urgent travel requires it, the consulate may issue a limited-validity emergency passport. The Department of State says that in most cases a replacement passport abroad is issued the next business day, although exact timing depends on the consular circumstances. Emergency passports are valid for up to 1 year.

For a U.S. passport application, you generally need evidence of U.S. citizenship, physical photo identification, photocopies of the required documents, one passport photo, and the applicable fees. For a lost or stolen passport, provide the relevant loss/theft information on Form DS-11 and, when applicable, Form DS-64. The Department of State accepts several types of citizenship and photo-ID evidence, so a certified birth certificate or driver's license is not the only possible combination.
